Description:
LIABILITY RELEASE:
I/We (camper's legal guardian) approves this application and certify that my child is in proper physical condition to participate in this program. I/We grant permission for my child to participate in all camp activities.
I understand that no refunds will be given in the event that my child leaves early due to sickness, homesickness, or disruptive behavior. I/We understand that Camp Clemson is not responsible for lost, stolen or damaged personal articles.
I hereby release City of Clemson Parks and Recreation, the Central Clemson Recreation Center and any employees associated with this program from full and complete liability or claim of injury or damage sustained by my child resulting from participation in this program. Finally, I understand and agree that this release and waiver includes any claims based on actions, omissions, or alleged negligence, gross or otherwise, of the City of Clemson, and/or their elected officials, employees, agents, representatives, volunteers, invitees, guests, participants, contractors, and referees/umpires.
REGISTRATION TERMS AND CAMP SPECIFIC POLICIES:
- I understand in order to register for CAMP CLEMSON you must pay 1 weeks fee + the $40 registration fee.
- I understand that FULL TUITION cost for each session is due Wednesday the week prior to attending. I understand that no payment is refunded for missed days regardless of the circumstances. (If student is issued a quarantine we will honor refunds for those dates)
- You may cancel sessions if you do so 1 week prior to attending. If cancelation occurs 5 business days or less prior to the Monday of the week attending the tuition will not be refunded and the charge will not be removed from the account.
- I understand that scholarships are available for families that qualify for free lunch or reduced lunch. To apply for this scholarship please email staff @ campclemson@cityofclemson.org
- Pick up is to occur no later than the close of camp. Parents will be charged $10.00 per day after 5:35PM. If late pick up occurs more than 3 times the student will be removed from the program. *See hours of operations for ALL SITES for information on drop off and pick up times.
- I understand that return checks are subject to a $30.00 fee.
BEHAVIOR POLICY AND TERMS:
The number one goal of our policy is to keep all children in the program safe. Please
review our policies below so you fully understand the system we use for behavior infractions.
1.Verbal Warning
2.Written Warning- parent signature required
3.Written Write Up- parent signature required
4.Dismissal.
*Please discuss the write up system with camper before attending.
Behaviors that constitute immediate dismissal:
For severe offenses, such as but not limited the list below, participants may be immediately dismmised from camp
- fighting/hitting
- theft
- vandalism
- bullying
- possession of weapons
- possession of drugs
- severe verbal threats
- sexual misconduct
- any aggressive physical contact or related behavior
Examples of behaviors that constitute a write up:
*Please note that unsafe and overly disruptive behavior is not tolerated and each infraction will be looked at independently and at the discretion of the director.
- Walking away from camp staff
- Disrespecting camp staff / defiance of staff instructions
- Crawling under bathroom stalls
- Climbing any structures not authorized by staff
- Calling of names, bullying- depending on severity
- Swearing
Participants will not be disciplined in camp for behavior that occurred outside of camp hours,
even at parental requests.
MEDICATION POLICY AND TERMS:
- Medications should be administered at home whenever possible. All medications to be administered during program hours must have parent/guardian authorization. The parent/guardian must deliver the medication to the camp location and give to designated staff.
2. The first dose of any new medications must be given at home.
3. All medications must be properly labeled with the child’s name, name of medication, exact dosage to be taken, expiration date and exact time or frequency dose is to be taken. The medication must be in the original container with the prescription label or direction label attached. The form and container must match. Make sure medication has not expired and will not expire during camp.
4. Medications may not be accepted by personnel unless the Medicine Administration form is completed and signed.
5. The Parent/Guardian is responsible for submitting a new form each time there is a change in dosage or a change in time which medication is to be administered.
6. All medication is kept in a locked area only accessible to authorized staff.
7. When an authorization for medication expires, the parent/guardian shall be notified that the medication needs to be picked up within 7 days. Any medications that are not picked up by the parent within 7 days will be destroyed.
8. Camp Clemson does not assume responsibility for unauthorized medication taken independently by the child.
9. Under no circumstances may any staff member facilitate the taking of any medications outside the procedures outlined here.
IN ORDER TO REGISTER AND PAY FOR CAMP, YOU MUST AGREE TO THE POLICIES AND TERMS LISTED ABOVE.
The contact will be automatically added to the class as a registrant if someone drops out from a full class.
